Getting Started

 A  written referral from a primary care physician is necessary before a child may be evaluated. Doctors, therapists, school systems, and early  childhood intervention programs often suggest such a referral.
Our office will fax this form to your physician upon your request.  

 We accept and file insurance, including Medicaid, for eligible children. We will also contact your insurance company to determine coverage for services.  

 Once a family contacts our clinic, we send a packet of information that includes a questionnaire to be completed by the family or guardian. After we receive ​the completed questionnaire, we schedule an appointment for an in-depth evaluation, which results in a detailed written report.
